MIS 021 Solved Guess Paper is a 22-question revision file for IGNOU course MIS 021, Safety Philosophy and Principles of Accident Prevention, written for the December 2026 term-end exam. Every topic was checked against six real papers, December 2023 to June 2026, and every answer comes straight from the IGNOU course book in simple English.

MIS 021 opens the first semester of the Industrial Safety programmes and sets the base for everything after it: why accidents happen, how fire behaves, how a safety department is organised and what an accident really costs a company. It runs across four blocks and 16 units, which is heavy reading for someone who already works full time.

1. What Is the MIS 021 Solved Guess Paper?

It is a set of 22 fully solved MIS 021 questions, chosen by how often each topic came in six IGNOU papers and answered only from the course book. Every answer card shows its priority, block and unit, marks split and the sessions in which that question was last set.

The logic is simple. Four blocks are too much to read cover to cover in the last month, so you revise the questions IGNOU keeps coming back to, written the way an evaluator likes to see them: a definition first, numbered points with the right count, a diagram where the question asks for one and a short conclusion.

This file sits inside our wider set of IGNOU Solved Guess Paper files, which use the same past-paper method for each course. If you have been searching for an IGNOU MIS 021 Guess Paper that has worked answers and not just a list of questions, this is that file.

2. MIS 021 Sample Questions with Answers

Below are 5 of the 22 answers exactly as they appear in the file, covering all four blocks. The other 17 are in the full PDF. Each sample keeps its question number from the file, so you can find it in the index further down.

Sample 1 of 5File Q1Block 2, Unit 710 marksAsked in all 6 papers

The 5S concept of housekeeping

Question: Describe the 5S concept of housekeeping. Why should we implement 5S?

5S: the concept

5S is a process designed to organise the workplace, keep it clean, maintain effective and standard conditions.

The tool was started in the 1920s by Henry Ford as the CANDO programme. It was made popular as 'Japanese 5S' in 1980 by Hiroyuki Hirano. In Japan it is also called 'workplace management'.

The five steps of Japanese 5S

  1. Seiri (Sort): Separate necessary and unnecessary items. Remove the unwanted items and put a red tag on them.
  2. Seiton (Set in order): 'A place for everything and everything in its place.' Label things so they are easy to find and put back.
  3. Seiso (Shine): Clean the workplace. Cleaning is also a form of inspection, so look for loose bolts, noise and heat.
  4. Seiketsu (Standardise): Make the first three S a fixed routine. Use checklists and visual management.
  5. Shitsuke (Sustain): Make 5S a daily habit and discipline. Use 5S posters, slogans and a 5S Day. This is mainly a management job.

Benefits of the practice of 5S

  1. Safe workplace: The workplace becomes cleaner, safer and well organised.
  2. Better space: Floor space is used better.
  3. Less searching: Time spent searching for tools and papers goes down.
  4. Fewer breakdowns: Clean machines break down less, so equipment life increases.
  5. Less waste: Errors, defects and material wastage are reduced.
  6. Higher morale: Employee satisfaction and productivity improve.

Conclusion: The general concept of 5S is to eliminate waste (Osada, 1993). It is simple, needs very few resources and makes the workplace neat, safe and productive.

Domino theory of accident causation

Question: Discuss any one theory of accident causation. Explain Domino's theory of accident causation.

For every accident that causes a major injury, there are 29 accidents that cause minor injuries and 300 accidents that cause no injury. (Herbert Heinrich, 1931)

Herbert Heinrich developed this theory in the late 1920s. His book 'Industrial Accident Prevention: A Scientific Approach' (1931) studied about 75 thousand accident cases. He found that 88% of accidents are caused by human errors, 10% by unsafe conditions and 2% are unavoidable.

The five dominoes

  1. Social environment and ancestry: Family background and social life shape a worker's skills, beliefs and character.
  2. Fault of person: Carelessness, bad temper, ignorance or recklessness of the worker.
  3. Unsafe act or unsafe condition: A worker error, like standing under a suspended load, or unguarded machinery.
  4. Accident: An event like a fall or being hit by a flying object.
  5. Injury: The result of the accident, such as cuts and broken bones.

When one domino falls, it knocks down the next. Removing any one domino prevents the accident. It is best to remove the middle one, the unsafe act.

Update by Bird and Loftus

  1. Management influence: Visible commitment and leadership of management.
  2. Other losses: Losses besides injury, like productivity loss and property loss.

Conclusion: The domino theory shows that accidents follow a chain. Break the chain at the unsafe act to prevent accidents.

Safety audit: how it is conducted and why it matters

Question: (a) What is Safety Audit? How is it conducted? (b) What is the significance of performing a safety audit?

(a) Safety audit and how it is conducted

A safety audit is a constructive process which examines the safety policies, programmes and practices within a work area.

Audit must be done efficiently and consistently by competent auditors. There are three major phases of safety audit.

  1. Pre-audit activities: Select and form the team, prepare a pre-audit questionnaire, review relevant regulations, define audit scope and team duties, review audit protocols and develop an agenda.
  2. Audit (on-site activities): The team tours the site, reviews records, inspects the facility and interviews staff. A preliminary copy of findings is prepared.
  3. Post-audit activities: Feedback response from the site, final report submission, and audit follow-up and closure.

(b) Significance of performing a safety audit

Safety audit checks whether methods and practices follow the laws. It finds the areas that need change and gives remedial actions. A safety audit is essential, as it will help to:

  1. Minimise injury: Minimise the risk of personal injury.
  2. Customer confidence: Ensure customer confidence.
  3. Avoid loss: Avoid material loss from a business.
  4. Morale: Promote employee morale.

When an organisation acts on audit results, the chances of injuries, illnesses and deaths go down.

Conclusion: Safety audit is a health check-up of the safety system. It shows weak points before an accident happens.

Personal protective equipment for head, eyes, face and feet

Question: Why are PPE required in industrial safety? Explain the various kinds of PPE used to protect the head, eyes and face, and legs and feet.

PPE are those equipments that protect the users against associated risks and possible dangers at work place.

The requirement for PPE

  1. Hazards everywhere: Chemicals, falling objects, sharp edges and flying sparks are present at work.
  2. Last line: When engineering and administrative controls fail, the employer must give PPE.
  3. Limit injury: PPE is not an accident prevention measure. It keeps the injury to a minimum.

Eye and face protecting equipment

  1. Safety glasses and goggles: Protect from dust, debris and chemical splashes.
  2. Welding shields: Filtered lenses protect from intense light and sparks.
  3. Laser safety glasses: Protect from laser light.
  4. Chemical splash glasses: For chemical and hot liquid splashes.
  5. Face shields: Cover forehead to chin. Wear goggles underneath for impact.

Head protecting equipment

Hard hats or safety helmets protect from falling objects, bumps, burns and electric shock. A helmet should defend against falling objects, be shock resistant, be water resistant and slow burning, and have a hard outer shell with shock absorbing lining.

Foot and leg protecting equipment

  1. When needed: Heavy objects falling, sharp objects, molten metal or chemicals, wet floors and electrical hazards.
  2. Guards: Leggings, metatarsal guards and toe guards.
  3. Special shoes: Electrically conductive shoes, electrically non-conductive shoes and foundry shoes.

Conclusion: The right PPE, with proper fit, training and care, keeps workers safe from job hazards.

Key factors in accident analysis and investigation forms

Question: (a) Discuss the various key factors considered in analysis of an accident. (b) Explain the various elements of incident investigation forms in brief.

(a) Key facts considered in the analysis of an accident

Investigation of the accidents is primarily regarded as a fact-finding procedure.
  1. Nature of injury: The type of physical injury. The more severe one is reported.
  2. Source of injury: The object or substance that directly caused the injury.
  3. Accident type: Based on recovery: first aid cases, home case accidents and lost time accidents.
  4. Hazardous condition: The physical condition that allowed the accident to happen.
  5. Accident cause: Unsafe acts (human actions) and unsafe conditions (physical conditions).
  6. Agency of an accident: The object or part of the premises where the hazardous condition existed.

(b) Elements of incident investigation forms

  1. Date and time: Exact date and time of the incident.
  2. Place and area: Exact spot, with maps and sketches.
  3. Employees injured: Names, jobs, type of injury and body part affected.
  4. Damage to property: List of machines and property damaged.
  5. Description: Process details and step-by-step events.
  6. Photographs and videos: Pictures and sketches of the scene.
  7. Interviews: Of injured employees and witnesses.
  8. Supervisor acknowledgement: Supervisor and management confirm the incident.
  9. Root cause analysis: The actual cause, and the most important part of the form.
  10. Preventive steps: Recommendations with expected completion dates.

Conclusion: Correct facts and a complete form help find the root cause, so the same accident does not happen again.

Happy with the style? The other 17 answers, including the fire triangle, Bhopal gas tragedy and safety culture cycle, follow the same format.

There is no lab record or practical to submit for MIS 021. Apart from the term-end exam, the only other component is one tutor-marked assignment. Q22 deserves a closer look: it holds eight short notes, from Swiss cheese theory to power arc blast, and the file's version of the question asks for any four.

5. MIS 021 Exam Pattern and Answer-Writing Tips

The MIS 021 term-end exam runs 3 hours for 70 marks, and the two latest papers ask you to attempt any seven of nine questions of 10 marks each. Older papers had a compulsory first question, so check the layout as soon as the paper is in your hands.

Your grade also includes one tutor-marked assignment with 30 per cent weightage, and IGNOU asks for at least 40 out of 100 in it. The rules are in the official MSCIDS assignment booklet for MIS 021.

No word limit is printed on the paper. The file works to about 250 words for a 10-mark answer, 150 words for a 5-mark part and 75 words for a short note. Draw a neat labelled diagram whenever the question says draw, because the fire triangle, organisation chart and safety culture cycle carry marks that plain text cannot earn.

Three habits help most. Give the exact count the book uses, such as five causes of fire, four levels of responsibility or six loss control techniques. Use a table whenever the question says differentiate. Stick to the book's own examples, like Bhopal, the Jaipur oil depot fire and Deepwater Horizon, instead of outside cases.

6. MIS 021 Syllabus: Blocks and Units

MIS 021 has four blocks and 16 units: industrial safety basics, safety engineering, safety management, and industrial accidents with their prevention. The table maps each unit to its main exam topics and to the guess paper questions that cover it.

If you want full unit-wise reading beyond exam revision, our MIS 021 Study Material follows the same block order, which makes it easy to move between the book and this file while you revise.

7. Which IGNOU Programmes Include MIS 021?

MIS 021 is studied in five IGNOU programmes: four in Industrial Safety and the MA in Environmental and Occupational Health. The course code and term-end paper are the same across them, so one file serves every learner listed below.

  • PGDINDS: IGNOU Post Graduate Diploma in Industrial Safety
  • PGDINDSOL: Post Graduate Diploma in Industrial Safety (Online)
  • PGCINDS: Post Graduate Certificate in Industrial Safety
  • MSCIDS: Master of Science in Industrial Safety
  • MAEOH: Master of Arts in Environmental and Occupational Health

Most Industrial Safety learners are working engineers and safety officers, while many MAEOH learners come from health or environment backgrounds. That is why the answers avoid heavy theory and stick to what can be written in the exam. If you are on the MAEOH track, the MAEOH guess Paper set covers your other courses in the same format.

9. MIS 021 Guess Paper FAQs

Quick answers to what MIS 021 learners ask us most on WhatsApp.

What does the MIS 021 guess paper for December 2026 contain?

The file holds 22 solved questions from all four blocks of Safety Philosophy and Principles of Accident Prevention, spread over 22 pages of answers in a 25-page PDF. It also includes three labelled diagrams, exam-writing rules and a frequency grid covering six IGNOU papers. Five of the 22 answers are shown on this page, so you can check the quality before you order.

Which MIS 021 topics are most important?

The 5S concept of housekeeping was asked in all six papers from December 2023 to June 2026, and safety training techniques came in five. Cost of accidents, Domino theory, safety audit, PPE, hazardous material handling, accident investigation and causes of accidents each came four times. Revise these nine topics first, because together they cover most of the choice you get in the paper.

How many past papers were used for the MIS 021 guess paper?

Six term-end papers were analysed: December 2023, June 2024, December 2024, June 2025, December 2025 and June 2026. Each of 28 topics was ticked against every session to find repeats and recent gaps. The answers were then written only from the IGNOU MIS 021 course book, so the definitions and point counts match what evaluators expect in your answer sheet.

Which IGNOU programmes study MIS 021?

MIS 021 is part of the Post Graduate Diploma in Industrial Safety (PGDINDS), its online version PGDINDSOL, the Post Graduate Certificate in Industrial Safety (PGCINDS), the Master of Science in Industrial Safety (MSCIDS) and the Master of Arts in Environmental and Occupational Health (MAEOH). The course code and exam paper are shared, so one guess paper serves every learner in this group.

Does MIS 021 have a practical file?

No. MIS 021 is a theory course, so there is no practical file or lab record to prepare for it. Your grade comes from one tutor-marked assignment, which carries 30 per cent weightage, and the three-hour term-end exam. That is why this page shows the full 22-question index of the guess paper instead, so you know exactly what the complete file covers.

Which diagrams should I practise for the MIS 021 exam?

Practise three: the fire triangle with heat, fuel and oxygen, the safety organisation chart with its four levels of responsibility, and the seven-step safety culture development cycle. Questions that say draw, or ask for a neat diagram, lose easy marks without them. All three are drawn in the full file in a simple style that you can reproduce neatly within a few minutes.

How long should an MIS 021 answer be in the term-end exam?

No word limit is printed on MIS 021 papers, so the file works to a simple target: about 250 words for a 10-mark answer, 150 words for a 5-mark part and 75 words for each short note. Start with the book definition, give numbered points with the exact count, add the diagram where asked and close with a two-line conclusion.

How can I get the full MIS 021 Solved Guess Paper?

Send a WhatsApp message to 9899436384 with the course code MIS 021. Our team replies with the price and payment details, and the complete 25-page PDF with all 22 solved questions reaches you on WhatsApp after payment. If you also need other Industrial Safety courses such as MIS 022 or MIS 024, list the codes in the same message so everything comes together.
